I’m going to use this to show my doctor and see if I can get diagnosed with anxiety and get a doctor’s note, and to get accommodations from my university. It’s a very well written letter. I thank Jennifer Jo-Brout for writing this.

Hello. I have an apt. with my Dr. next week, and I’m trying to find some information as I need a letter for college to receive an accommodation for test taking. This letter is not available anymore. Could you please update this link?

I would very much appreciate the sample letter to take to my psych doctor. I had appointment yesterday and informed him my manager wants a note from him stating I need accomadations. Mainky to not work with several people that are triggers for me. Example: gum slpping with mouth, cracking gum, humming constantly, tapoing their clonky shies, rustling candy papers in picket all day. Its vvvery debilitating for mmme but I need to keep my job.
